Jefferson Parish Assisted Evacuation Assessment RegistryTHE JEFFERSON PARISH SPECIAL NEEDS REGISTRY IN NO WAY REPLACES THE RESPONSIBILITY OF INDIVIDUALS TO HAVE THEIR OWN EMERGENCY PLAN This registry is for assessment purposes only. This information will assist Jefferson Parish in obtaining the number and needs of citizens in the Parish that may need Public Assisted Transportation during an evacuation.
Very Important:All registrants are personally responsible for utilizing the public transportation system to obtain transportation to one of the parish's pick up points for evacuation out of the danger zone.
Please Note:The registration of this form will not generate a call or pickup from Jefferson Parish during evacuation. This form is for informational purposes only and in no manner a guarantee of assistance. When the time comes to implement the publicly assisted evacuation plan, government officials will announce on television and radio the phone numbers to call and the locations of pick up points for citizens to utilize for evacuation.
The function of this list is for planning purposes only. You will not automatically be picked up in the event of a mandatory evacuation. All evacuees who have mobility limitations and /or a disability that prevents them from using the JET Buses will need to call the Special Needs Hotline that is released during an evacuation to schedule a pick up. All others will need to go to one of the published bus stops for transportation.
SPECIAL NEEDS INDIVIDUALS GOING TO A SHELTER WILL BE ACCOMPANIED BY THEIR PERSONAL CARE-GIVER
